preparing to unleash Zombie Titan TV Man's beast form
The Plague Doctor, also officially called SCP-049, is the true main antagonist of the series being the former main antagonist in Skibidi Toilet Zombie Universe Arc 1 and the current overarcing antagonist in Skibidi Toilet Zombie Universe Arc 2 , being responsible for causing the Virus-19 outbreak and the rapid downfall of both the Skibidi Toilets and Alliance. He is shown to be extremely powerful, as he is able to subdue both the Detainer Astro Toilet and G-Toilet X with ease.
The Plague Doctor is a dangerously intelligent and cunning entity capable of causing wide-spread destruction through the usage of Virus-19, an infection that affects both living tissue and hardware. Despite his rather powerful abilities, he prefers to confront opponents alone when they have no means of assistance from their allies.
His plans are further supported by a loyal servant of his, the Rogue TV Man, who works as a double agent within the ranks of the Alliance. Although not shown directly, the Plague Doctor is implied to be the one responsible for the captivity of both the Titan Cameraman and Titan TV Man, as they are the most dangerous and powerful beings that could easily destroy the Plague Doctor's larger infected.
During the events of Episode 30, Plungerman would successfully trick the Plague Doctor into grabbing a decoy USB that Plungerman had swapped the Virus-19 USB with. After the doctor had crushed the decoy USB with his hand, Plungerman soon retaliated by punching him with such force that it resulted in half of the doctor's plague mask being shattered, exposing one side of his face as a result. It would then be revealed that the Plague Doctor was actually Dave, Plungerman's previous human state before becoming a Cameraman.

Appearance[]
The Plague Doctor takes on the appearance of a 13th century bubonic plague doctor. However, instead of just wearing a long black waxed gown, the Plague Doctor wears it alongside a black cloak which is complete by its iconic plague bird mask. Additionally, the Plague Doctor wears a brown harness below his cloak, presumably for carrying equipment and to hold his clothing in place.
Combat/Abilities[]
The Plague Doctor is a very powerful entity, being responsible for the entire infection in the first place. Said infection was initially spread through hardware to software, with the Plague Doctor sabotaging the Chief Scientist Skibidi Toilet's experiment to mind-control Alliance members through swapping the mind-control USB with the Virus-19 USB. Though, it is unknown whether the Plague Doctor is capable of infecting/killing organisms through physical touch, as MonsterUP refers to the Plague Doctor as SCP-049; which is capable of killing people through touch.
Similarly to the TV Men, he is capable of teleportation, presumably an ability gifted to him by the Rogue TV Man. Furthermore, he seemingly possesses the ability of telekinesis, being able to abruptly stop any attackers from reaching him to cause harm. This ability is shown to be extremely powerful, capable of stopping the Detainer Astro Toilet and the G-Toilet himself, which keep in mind, is a titan-sized creature.
This telekinesis ability is used in unison with the ability to drain the energy from living organisms, which has shown to have detrimental effects on aforementioned organisms, practically leaving them unable to fight nor resist.

- The creator of the series, MonsterUP, refers to the Plague Doctor as SCP-049, making him linked to the SCP Foundation mythos in some form of way.
- Despite this, Plague Doctor is not a 1:1 adaptation of SCP-049, as he is later revealed to be an evil Plungerman from an alternate universe where the SCP Foundation exists, and does not share the principles, powers, goals and vague origins of the latter.
- Due to him being a Plungerman/Dave variant, he is able to use the 4 Cores.
